History

In June 2001, the Manila Doctors Hospital (MDH) administration approved the creation of the Department of Family and Community Medicine (DFCM) to enhance the Emergency Room (ER) services, further uplift its quality health care system, and to contribute to the corporate social responsibility through community services. The following year, the DFCM commenced its training program with two (2) pre-resident Trainees.

In February 2002, the Specialty Board of Family Physicians of the Philippine Academy of Family Physicians, Inc. (PAFP) accredited DFCM's Training Program. The department received level accreditation in 2003 and Level by 2004.

By the middle of 2004, the Department had signed two Memoranda of Agreement (MOA) with the Philippine General Hospital to augment training of DFCM Residents in Psychiatry and Behavioral Medicine; a similar MOA was forged with the Ospital ng Sampaloc for Obstetrics¬Gynecology training.

The community rotation started in October 2001 at Our Lady of Peace Hospital along Coastal Road and in Sitio Puling Bato in Navotas the following year.

In 2003, the community rotation transferred to Barangay 178, Maricaban, and Pasay City with a population of more than 5,000 and 1,060 households.

Department Services

The Department of Family and Community Medicine of the Manila Doctors Hospital provides the following services OPD consultation and admission, ER consultation and admission, direct ward admission, community and family health and wellness services.

Emergency Rooms

  • ER Consultations and Admissions:
    • All patients who consult at the Emergency Room are seen by DFCM intern and resident rotator.
    • After obtaining pertinent history and PE, the resident provides adequate treatment and management and participates in decision-making on proper referral network.
    • Admissions under the consultants of Department of Family and Community Medicine are admitted by the DFCM resident on duty.
  • Direct to Room Ward Admissions:
    • All patients who consult at the clinic of affiliated DFCM consultant are provided with direct to room orders by attending physician.
    • Admissions under DFCM consultants are seen and examined by the OFCM residents and interns on duty.
    • The DFCM resident-on-duty, as well as the nurse-on-duty, shall inform the Consultant-in-charge of the admission.

Outpatient Consultation

  • Organization and Administration:

    • Objectives
      • Primary: To provide OFCM residents and interns in an exposure to the basics of outpatient services and management.
      • Secondary:
        • To gain knowledge in the outpatient procedure and policy-making
        • To develop he skills of proper referrals of outpatient cases.
        • To develop the awareness and skills of possible research.
        • To provide proper screening of candidates for social services (SS) cases.
        • To formulate possible projects in relation to outpatient management.
  • Staffing:
    • Supervisor /Head Nurse
    • Nurse-in-Charge
    • Clerks
    • Nursing Aids
    • OPO Coordinator
    • General duty: supervises residents and interns rotating in the OPO
    • Specific duties:
      • Plans, implements and monitors schedule of activities of OPD
      • Coordinates with the evaluation of rotators in OPD
      • Coordinates with other departments
      • Acts as a liaison between the department, administration and Ops.
      • Submits a monthly progress report of activities.
      • Submits a yearly budget.
      • Monitors the budget utilization.
      • Makes an annual report.
  • OPS Residents:
    • The clinical assessment and management of the patients.
    • The referrals of problematic patients to other specialties; with specialty services provided for on scheduled days.
    • The emphasis on patient's compliance and follow- ups.
    • The reporting of any problems encountered with their senior and for clearance with the OPS coordinator.
    • Cooperates with the OPS coordinator for making of the action plans, monthly progress report of activities, monthly census and annual report.
  • Family Medicine Consultant of the Week - Consultant whom the residents refer to for any problems encountered with their patients.
  • Patients' Records - contains confidential files of patients' complete history and PE, impression and management, progress in follow-ups. This is kept by the personnel OPS.
  • Guidelines:
    • Clinic Days: Daily (Mondays to Fridays) at 3:00 to 5:00 p.m.; except on Holidays.
    • Manned by Family Medicine Residents and Interns
    • The Family Medicine Intern will see the patient first and do the history and PE.
    • The Family Medicine Resident will then confirm the history and PE.
    • AU patients are logged-in in the DFCM-OPS logbook.
    • Follow-up should be made by the patient to ensure continuity of care.
    • Patients who will be seen in OPS will come from:
      • OPS itself (walk-ins)
      • Emergency Room
      • Department of Family and Community Medicine (DFCM) Consultants, active and courtesy
      • Adopted Community (Barangay 178, Maricaban pasay City)
      • Previous Adopted Community (Sitio Puting Bato, Navotas)
      • Other Consultants
      • Social Service Patients who come for follow-ups
