- Manila Doctors Hospital’s Wellness Hub Representative will send email to patient as confirmation of appointment.
- For safety purposes, patient will receive via email a copy of COVID-19 screening and health assessment form to be filled out and emailed back to wellnesshub@maniladoctors.com.ph.
- Clearance will be sent via email and patient to proceed to Manila Doctors Hospital NTMT2 Tower – Kalaw Lobby and proceed to Wellness Hub (9/F) on scheduled date of appointment.
